I’m replacing the anchor locker drain tube, I see no evidence there was ever a clam shell vent( if that’s the correct term) over it on the outside of the hull. Should there be?
I checked my bilge drain tube on the starboard bottom and it has evidence there was a clam shell vent there, so I’ll install a new one.

I would say so.
It's the larger of the two Perko clam shells. Whaler drilled 2 extra holes in it for a total of 5.

My 1987 Outrage 18 had no anchor locker drain clam shell so I added one.

Both anchor locker drain tube and rigging tunnel sump drain tube have the Perko 2x2 chrome plated brass clam shell vents installed on the bottom with 5 truss head screws at the factory. I don't remember the screw size, maybe #8 1/2". Last year Perko started making these in stainless steel, which is what I installed.

— #8 x 1/2-inch truss head screws
— clam shell vents with five holes
— clam shell vents with three holes

Perko clam shell vents used with through-hull drains are mentioned in the FAQ:

The covering of the through-hull drain outlet is usually only done if the drain outlet is on a running surface of the hull. Bow locker drains that exit the hull well above the water line may not have a vent cover. Use of the vent covers on particular drains on particular models may have varied in various production epochs.
If a drain were used, there ought to be very clear evidence: three to five fastener holes.
