I'm dense, can someone explain this to me?

The two regions in use today (and their bouyage characteristics) were adopted to replace dozens in use previously.

I don't know why they didn't just pick one. Probably for the same reason some countries drive on the wrong side of the road...

OK, to simplify it, is this correct:

In some parts of the world it's RED RIGHT RETURNING, with some exceptions for the Intercoastal

In other parts of the world it's GREEN RIGHT RETURNING ??

For the most part, I can say yes. I've only a dozen or so hours on the ICW, so I can't speak to any exceptions.

In regards to your exam question, you should know: there are two bouyage systems in use today IALA A and B. Your charts will tell you which system is depicted on the chart. You must be familiar with the conventions and characteristics of each (to pass such an exam).

I don't boat much in IALA-A, so even though navigation and charts are a very small hobby of mine, I don't concentrate too much on them.

For me, inland navigation is much more challenging.

In reference to exceptions on Red Right Returning on the ICW you may find this interesting.

"With a few exceptions, the intracoastal waterway (ICW) has red daymarks on the side of the mainland shore. Thus, on the Atlantic Coast the red daymarks are on the starboard side when heading south. The opposite would be true if your are heading south on the west coast of Florida. The exception is when one is heading south on the Atlantic Coast and out a channel. In this case, the yellow symbol on the intracoastal waterway daymarks must be used to determine the direction and position of the ICW. The daymark shown on the right is sometimes found at the junction of a channel and the ICW. It should be kept to PORT for vessels following the channel inland from the sea. However, it should be kept to STARBOARD for those vessels following the ICW from New England to Texas (and kept to PORT for those traveling the ICW from Texas to New England)."

In our area there are a lot of rivers, creeks etc. merging into the ICW. If you aren't paying attention to the yellow symbol for the ICW you could end up where you don't want to be.

Just to confuse you more there are places where the even/red/triangular ICW markers have a square gold ICW decal, and there are odd/green/square ICW markers with triangular gold ICW decals. Basically while operating your vessel in the conventional direction of bouyage the stbd ICW marker is a green navaid and the port ICW marker is a red navaid.
